Microangiopathic Homolytic Anemia in Patients with Gastrointestinal Malignancy

Abstract
Microangiopathic hemolytic anemia is characterised by helmolytic anemia, usually with
thrombocytopenia, and presence of fragmented red cells in peripheral blood smears.
Microangiopathic hemolytic anemia and thrombocytopenia is well-described complication
of widely metastatic carcinoma. Two patients with metastatic adenocarcinoma of
stomach and one patient with adenocarcinoma of colon developed microangiopathic
hemolytic anemia and thrombocytopenia.
One patient underwent intensive plasmapheresis and had a clinical response. Serum
sialic acid level and MMADCC (monocyte mediated antibody dependent cytotoxicity) was
paralled by this response.
Two patient had a transient response after chemotherapy with 5-fluorouracil,
adriamycin, and mitoinycin. Speculation as to pathogenesis and appropriate therapy are
discussed.
